• Must be a resident of Defiance, Paulding, Putnam or Van Wert County
• Must not have been convicted of any violent crimes
• Must not have been convicted of any escape crimes
• Can not be a sexual offender or sexual predator
• Must meet with the EMHA staff once a week, on Tuesday
• Must have a stable, full-time residence
• Must be able to make weekly payments as determined by the Court’s approved sliding fee scale
• If unemployed, may not schedule times away from home to look for employment
• If employed, must provide proof of employment
• Must provide a weekly work schedule signed by supervisor
• Must provide a set schedule once a week on Tuesday
• Must maintain continuous phone service with no added features (call waiting, call forwarding, caller I.D., internet, etc.
• Must provide a valid Ohio I.D. or drivers license
• Must be willing to sign all documents and comply fully with all rules
